I will have to think about some of these things you have pointed out.
Some of the combinations of tags don't occur in Bookshare files.
The italic sign on during is a simple bug.
The case of </sup> followed by <strong> can be handled with a change in
dtbook.sem Find the line for sup and at the end add " \s" without the
quites. The documentation for liblouisutdml explains what this is.
