[liblouis-liblouisxml] Rules for handling emphasis in UEB and other codes

This is for the transcribers on the list. What is the sequence of 
Braille indicators? At present, liblouis places them after punctuation 
at the beginning, first emphasis indocators, then capitalization 
indocators. For UEB capitalization indicators would come first after a 
phrase, then emphasis, then punctuation. However, suppose the emphasis 
includes the punctuation, as in bold or underline? Suppose the only 
thing emphasized is punctuation? 

I'm considering two algorithms to handle Braille indicators. The older 
one may be inadequate. The newer one will require quite a lot of work 
and time to implement.
