Indeed, when using the Google toolbar, you must sometimes use shift-enter
to force the linked page to open in a new window.  Sometimes that does not
work either.  In those cases I try right-clicking on the link with the
right mouse button and choose Open Link in New Window.  Occasionally that
works.  If none of this works, you have two choices.  Perhaps the easiest
choice is simply to turn off the toolbar temporarily, alt-v, enter, g.  But
you can also left-click on the icon stating how many ads have been blocked,
and once you do that, pop-up blocking is disabled on that page but remains
enabled everywhere else.

Of course, ad sites are not excited about the proliferation of pop-up
blockers.  They will do what they can to render them ineffective.

And so it goes.
