Hi Kevin, I had a brief look using Jaws 12 and Word 2003. Have you turned on the Forms Toolbar? If not, you could try doing this by going to "View", "Toolbars" and scroll down to the toolbar called "Forms". What I found is that, if I went to the top of the document and pressed the Tab key repeatedly, Jaws then jumped reasonably well through the various form fields announcing what was to be filled in. I think there is one edit box near the end which didn't speak properly and then, just after the check box, I tended to get an error message to do with macros being disabled or something like that. My wife tells me that I had managed to enter the details in the correct place though which I guess is the important thing from your perspective. Hope this helps.
