[isapros] Custom HTML Forms

Has anyone managed to get custom HTML forms to work with ISA2k6?
I have followed exactly as per the help file but keep getting a "401:
Unauthorised" error. If I remove the custom HTML form option from the
web listener all works as per expected. I have some customers who are
desperate to create custom forms for Sharepoint publishing, but don't
seem to be able to get it to work at all....am I going mad and turning
into a unihomed ISA admin??? :-)
Is this a known issue/bug?
