OK- So, where does the ISA server come into play?  Does your ISP use it to 
publish SMTP on the Exchange Server?  Or are you directly connecting to the 
SMTP server from behind ISA via the firewall client?  Is it always the same 
2 Meg file or different ones you have tried?  How many clients have you 
tried it on?  Is it the fw client or SNAT?
What kind of connection is in use?  Can you give us any more information?

I have done it many different ways, even Client-to-ISA-to-ISA-to-Exchange 
and visa-versa with no problems.  Details would help!
