Blanket statement: except for the Microsoft-supported Small Business
Server edition of Windows Server with AD, Exchange, ISA, and SQL on
one box. :)

Frank, unless an ISA guru knows the solution on here, you could try
reinstalling ISA 2004, and then re-applying SP1 for ISA 2004.
