I was a bit curious about this because we used to have phrase navigation available. We didn't keep it because it wasn't remotely reliable in that, phrase mark up was different in every book. It is often, as you say, one whole side of a cassette, but sometimes, it's just actually a phrase, or anything between the two. In other words, it didn't seem to be a reliable uniform type of navigation, and so seemed more useful to implement more uniform navigation types.
