The TODOs were probably added by me to indicate that those entries don't 
refer to any license, but probably should.

I haven't looked into the code using the lib, but I would have thought 
we're using the PDFlib Lite version. The respective license would be 
src/libs/pdflib/doc/pdflib/PDFlib-Lit-license.pdf. We might still violate 
the license, though (e.g. the "Redistribution" section).

Either refer to both or just to the more liberal one.

Under "Copyright" either list both or abbreviate to "John Smith et al.". 
Refer to both licenses or copy both licenses into one file and refer to 
that.

Just GPL v2.

If they are identical maybe just create a new generic license file,
