Last month, we had to deal with a very similar issue. Our alarm messages were different. The primary indicator of a problem was that user displays could not successfully view equipment from one particular node. Kevin gives great advice that I wish I would have had back then. The only additional thing that we did that Kevin did not mention was to swap cables around (copper and fiber) in an attempt to isolate the problem. We did not realize then that we could have been contributing to the problem. We even replaced the P0960KA module (ours are not redundant). Curiously, the hardware still indicated problems but the operator screens began to work again...for about a week. In the end, I changed the only thing I had not changed before. I bypassed a passive tap and connected the P0969KA directly to the Copper to Fiber converter. All the errors on the B cable cleared up. It looked so good, I did it on the A cable too. The passive taps have a grounding strap on the outside. We believe that either the passive tap had become part of a ground loop or that it may have had a "noisy" ground caused by soft start motor starters in the local vicinity.
