As far as I can tell, IrfanView can open (and save) .ico files.  I can't
be sure it can save a .bmp as .ico in a way that makes sense though as I
have not tried that.

IrfanView can take BMP and PNG (Two filetypes I've tested) and export the
final results as .ico files.

Obviously the BMP or PNG needs to be edited to conform to the size and
8bit or less res requirements of the .ico files.
