Actually, for my records, I always use the year-month-day, but that hasn't really fully caught on here in Canada yet.

Date notation confuses me, as going through patients' old lab and X-ray results I see both numeric systems used. For years I have written simply (and for example), "Oct. 1/12", or the mil version: "1Oct12". Nice to know someone is equally vexed.
