I have CP Form 19 which are 6 1/2 " wide by 9 3/4" long, both lined and
unlined from the 1960s. An earlier pink form 19 (lined) was 6 1/2" wide by 7
3/8" long.
A Form 19A (pink) from 1946 is 6 1/2" wide by 14 3/4" long and is lined.
A Form 31A (white) is the same size as above and has places at the bottom for
signatures of the Conductor, Engineer, train, made, time and Operator
A NYC form 31 from 1960 is 6 1/2" wide by 9 3/4" long.
