The U.S. Fire Administration has received notice of the following
firefighter fatality:
Name: Phillip Stephen Hulen
Rank: Firefighter

Status: Volunteer
Years of Service: 2
Date of Incident: 04/06/2004
Time of Incident: 1856hrs
Date of Death: 04/06/2004
Fire Department Name: Vann Crossroads Fire Department, Inc.
Fire Department Address: 4550 Church Rd., Newton Grove, NC 28366-7312
Fire Department Phone: 910-594-1093
Fire Department Chief: Jamey Jones
Cause of Death: While en route to a department mandated training class,
Firefighter Hulen lost control of his vehicle and crashed resulting in his
death. No seat belt was in use.
