Hello HCCN PCMH Peer Group members,

Attached is the HCCN PCMH Matrix updated as of today (06/11/2014). We send our 
congratulations to Asian Pacific Health Care Venture as they received their 
NCQA PCMH Level 2 recognition yesterday!

Again, if you have any updates from your health center re: its PCMH 
transformation status, please let me know so I can update the Matrix.
