[brailleblaster] New Features in liblouisutdml

It was noted in private correspondence that the configuration setings in 
liblouisutdml did not support handling of  emphasis. A new configuration 
setting, emphasis, has now been added. Any  and all of italic, 
underline, bold and computer_braille can be specified for inclusion in 
the output. the work all is also accepted, meaning that all emphasis in 
the document should be included. Actual dot patterns for the emphasis 
indicators must be specified in liblouis tables. 

This should make the liblouisutdml configuration settings sufficient to 
handle all template requirements. in addition, special settings for a 
document can be included in a call to liblouisutdml or even in a <meta> 
tag.

The format setting for PEF has been added, but  not yet implemented.

BrailleBlaster will have to specify embossers in some way other than 
liblouisutdml configuration setings, since embosser drivers are 
deliberately not included in liblouisutdml.
