Robert,
It would probably be difficult most of the time to match up what someone
entered on the form and and the title and author which sometimes ends up in
the collection since they want us to add the series name to the title and
things like (editor) to the name in the author field. I expect they'll rely
on volunteers to check that.

Gerald, I wonder after a person fills out the form if the web site can tell
the submitter if the book is already in the collection and the ratting of
the book or the person can go and look it up.
