Is there a reason that I don 't know that you can't underline the first word? 
i.e., it won't translate to Braille or sound?

I did, in a recent book,put  the line on top of the hyphen  (I hit it 5 times) 
times underneath something (I forget what it was now. It didn't really show up 
until I bolded it.  It may be in a book I haven't finished proofing so I 
haven't checked it in and don't know how or evenif it will show up when the 
book gets into the collection
