It's possible that it hsns't been registered with the
U.S.copyright office--or that it has been but the
processing isn't complete yet.
It seems to me that as long as you have the info from
the copyright page, that's the info you can put in the
submission form. To be safe, I'd paste a copy of the
copyright page into the Comments space.
So you have the copyright name and date, and I see it
was published in the N.Y. (I also checked amazon since
you didn't say), so there should be no problem.
It's interesting: I've seen copyright holders be "the
estate of" or relatives or heir of an author, but I'd
never seen one as explicit as this--trustee of Chomsky
Grandchildren Nominee Trust.
