Hi Judy,
Our summer reading program is divided for K-5, and 6-8, and runs from the
last day of school to the first week of school.
Our K-5 students record the books they read and a sentence about them, then
turn in the paper logs to the library, where we count them up, and I keep a
running tally of who read how many books. We put out a "suggested summer
reading" list of books we liked this year divided by grade interest level,
and hand out the paper forms to teachers to send home in their homework
folders the last two weeks of school and promote it in our library classes.
For middle school it's a google form that they fill in for each book they
read, with the same information as K-5 paper logs, just digital. The MS
puts out its own summer reading list, and I get them to link to the form on
the summer homework push page that goes to all MS families.
All students who read 5 or more books get a free dress day (big motivator
when there's uniforms), and we usually have a "prize" book selection for
the student(s) who read the most in their division, and a choice from the
"prize bucket" (mostly tiny shaped cute erasers) for 2nd tier readers
(usually in a range - more than 15 but less than the kid who read the
MOST). There's also the mythical "if your whole grade participates you'll
get a snow cone truck/ice cream party/special treat" that has only been won
once 6 years ago, and never again.
Last year we had 188 students read 1,305 books. I keep our lists and
metrics year to year so I can keep an eye on participation.

1000 minutes by the end of the summer gets a certificate, a book pass to be
able to check out double the number of allowed books, and a B&N gift
certificate.
We usually only have a handful of students who participate.
