Sure, here is a recent note from a group I'm doing. The patient specific
information is tailored to each patient for each group session and the other
parts are consistent across group members in my documentation.

MH-Group Psychotherapy

 

Group Name: PTSD Psycho-education and Trauma Processing Class

 

Group Description: This group is for Vietnam vets with PTSD and focuses on
psycho-education about PTSD, followed by trauma processing. In today's
session, we wrapped up the formal instructional phase of the group and are
now transitioning to combat trauma processing.

 

Group Process/Dynamics: Group members continue to display good cohesion, and
several members of the group stated that they feel trust in the others in
the group and are now ready to move on to the combat trauma processing phase
of the group.

 

Group Theme(s): Today's group topic was "Finding Meaning." The group
discussed an exercise of seeing how much civilians in their lives actually
know about the VN war. The group had collaboratively generated a set of what
they deemed to be very basic questions about VN which were then presented to
others in their lives in the loose format of "Are you smarter than a 5th
grader (about the VN war)?" Group members presented information on how much
accurate information is actually known to people who were born after the VN
era. The instructional part of the group was then completed with a
discussion of the general plan for the next phase of the group (e.g. talking
about trauma experiences, identifying potential "recipients" of trauma
letters, writing the letters, reading them in the group, and processing
them). We also reviewed the purpose of the letters (e.g. exposure,
consolidating and processing memories in a cohesive narrative, collective
grieving, and bearing witness to losses that have been sustained by members
of the group).

 

Patient Specific Information:

PT was talkative and participatory as usual during the group. He spoke about
the results of his effort to see how much his close family members actually
know about VN and stated that he realized how little he talks about VN, even
to close family members. PT also talked about the "stolen valor" movement
and how it feels appropriate to levy harsh penalties on those who
misrepresent themselves as having served in combat actions.

 

Diagnosis: PTSD

 

Plan: The group has now transitioned to a more free flowing structure to
accommodate sharing and processing combat trauma memories and losses through
discussion and readings of trauma processing letters written by group
members.
