atw: form development in MS Exchange

Hi, I have a client who wants to develop a small number of customised
forms developed in Outlook (MS Exchange Server). For example, they want
to create a Purchase Requisition form, which gets its info such as:
- Requestors Name: (from Global Address Book)
- Ph: (from address book)
- Items to be purchased: (freeform text)
Etc.
If anyone has the necessary skills to develop in this environment,
please give me a call
