Just to give a bit more context...
The project already has a change management plan - its policy & processes are
already defined.
In change management (CHM) there are 3 main types of change - normal,
emergency, and standard.
Outside of process is the operations guide. This guide says 'this is how you
use the process'; things like how are changes approved, who are the approvers,
what constitutes an emergency change, what SLAs are in place; to name a few.
The guide links to work instructions for the toolset.
We are now writing the operations guide and it’s the organisation of the
information in this guide, I was asking about.

Subject: ITIL change management documents
Hello, is there anyone on this list who has managed (defined, created, or
assisted in) a suite of documents for change management in an IT environment.
My tech writing career has skewed into ITIL (I have ITIL foundation
qualifications) doing process mapping & writing processes, procedures & work
instructions.
I am working in an area whose responsibility it is to write change management
operational documents - not the process documents.
The reason for this email? I am keen to see the titles and contents lists of
each document in a change mgt suite of documents. Our writing project hasn’t
been defined and I would like to see how it has been done elsewhere (it takes
too long to reinvent the wheel).
The industry I’m in is telecommunications.
I am happy to be contacted off-list to provide more information.
