Deb:
If you just google "change management document templates" and explore a bit
in the results, you might have yourself a few simple starting points...

Hello, is there anyone on this list who has managed (defined, created, or
assisted in) a suite of documents for change management in an IT
environment.
My tech writing career has skewed into ITIL (I have ITIL foundation
qualifications) doing process mapping & writing processes, procedures &
work instructions.
I am working in an area whose responsibility it is to write change
management operational documents - not the process documents.
The reason for this email? I am keen to see the titles and contents lists
of each document in a change mgt suite of documents. Our writing project
hasn’t been defined and I would like to see how it has been done elsewhere
(it takes too long to reinvent the wheel).
The industry I’m in is telecommunications.
I am happy to be contacted off-list to provide more information.
