That was almost 60 years ago when I read that as a preteen, so if it's not
in the V-2 book the only other possibility of related books that I can
think of would be the Willy Ley book- Rockets, Missiles and Space Travel, a
great book which inspired me to pursue rockets as a career.
As far as learning from mistakes, it would be a mistake of not wearing
safety glasses, that mistake cost me an eye, though I was a minor under the
supervision of two adults.
You don't think it will happen to you until it does.

Yes, you get a3way with something many times and you think it's safe
or the reports are exaggerated. Until it happens to you. Nitrous
Oxide is a big one which is handled routinely daily but has killed
people including a friend of mine.
On safety issues, it's important to learn from other people's
experiences, not only your own, because you might have just been lucky
(so far).
