I have had exactly the same problem from time to time. I never pinned
down the exact cause, but I have a feeling that either the length of
the original filename, or certain characters in the filename was
relevant.
Unfortunately I never thought to save the files to see if the "bug"
was repeatable after a reboot.
Searching Google for risc os alt click select rename bug does find
some pages where people have described this "bug" but I have never
seen a response that identifies the cause.
