[amayausers] Sample Run Sheets

Ok, time to pick some brains. We have had several mistakes due to
misunderstandings on the order sheets. Does anyone use a "run sheet"? Or do you
use two sheets, one to actually place the order for the item, and one showing
the exact work wanted. We do alot of things with a mixture of twill letters,
embroidery, screenprinting or transfers on the same item.
Any suggestions?
