Nope, but I used it today! Happy to let you know the outcome. The order for 
hearing I received was a joke- no wet ink signature, no affidavit of facts, no 
rebuttal to my affidavit. So I rejected the offer to contract using stamps, 
sent a modified version of this template to the court  and dropped a copy with 
the atty. 
I got the template from an organization called I self law am master who has 
been helping me in creating an estate trust(s) and becoming a secured party 
creditor. I know this group leans more into the patriot camp, but I'm of the 
mindset of David Straight- being able to operate/move well in all 3 
jurisdictions.
