[access-uk] aplication unknown

Hi,
I wonder if some one can help me, I keep getting this window up on my iMac,
Application unknown. I have had my wife look at the screen but their seems to
be nothing their., but what ever it is it blocks my computer and the only way
to get rid of it is by pressing VO D or VOSHIFT D, can anyone tell me what is
