Thanks for that, I made such enquiries with regard to a CVI when my local social security office in Northern ireland where seeking such, however neither the hospital nor the local authority here seemed to know what a CVI was, much less what information was required to be included so i eventually just sent in a bunch of test reports and told them ( the social security office ) to work it out from that and stated my reasoning.
funny thing is, a letter from my specialist at the hospital i attend carried more wieght than a comprehensive visual feild chart from my optician, ( however I don't think the feild test was properly conducted another story entirely).
Anyway. long story short the matter that needed resolving was eventually satisfactorily resolved..

This appears to be a good question because as far as i iknow there is no
standard form of cvi, as for the BD8. where in the dickens does one get a
hold of that?

From the website, it looks as though you need to get a bespoke proof
of disability form signed by a competent body/professional, but I was
wondering whether that was actually the case in practice.
