Here's the two that I use the most; I have no clue as
to their regional availability.
My #1 preference -> www.callatg.com <- $169/mo for
384k, $19/mo for a business phone line, and that
includes the equipment lease as well (a combo
modem/router/channel-bank unit). No, I don't work for
them, but I use them a lot because of their price &
reliability.
My fallback -> www.eschelon.com <- I have some clients
using them, $400/mo for a 512k, if I remember
correctly. That does not include the equipment,
although they're happy to sell you some.
