Zatim se tu pojavljuje GP-koji ispunjava svoj formular o pacijentu-FORMULAR MU PLAĆA ZA PRVI PUTA DVA(will pay the appropriate fee to your GP (doctor) for the completion of your medical form(s) on the first occasion only )-a za ponovljene potrebe PLAĆA PACIJENT. Failure to notify DVA is a criminal offence and is punishable by a fine of up to £1,000. Ida <http://www.nidirect.gov.uk/index.htm> n i direct government services The role of the Medical Section within Driver & Vehicle Agency (DVA) is to promote road safety by establishing whether drivers who have medical conditions can satisfy the medical standards of fitness required for safe driving. When will DVA investigate a medical condition Medical enquiries will need to be undertaken if any of the following circumstances apply: * one month after a stroke there are residual symptoms causing limb weakness, or visual disturbance, or problems with co-ordination, memory or understanding * epileptic attacks of any kind have occurred, other than those within the 24 hours following the stroke event * brain surgery was required as part of the treatment for the stroke event * there has been more than one stroke event within the past three months * your doctors have expressed concern about your fitness to drive * you hold either a current Large Goods Vehicle (LGV) or Passenger Carrying Vehicle (PCV) licence You'll need to contact DVA if any of the circumstances listed above are relevant to your case. If you currently suffer from any of the medical conditions or disabilities listed on this page, you will need to tell Driver & Vehicle Agency (DVA) about these. To do this you will need to complete a DL1 application form and forward it to DVA, who will then send you the appropriate medical questionnaires to complete. GP fees If you receive a medical questionnaire(s) as a result of notifying DVA of a condition, you will need to arrange for your doctor to complete the form(s). Your questionnaire(s) will include a consent form that must be signed. This allows DVA to make medical enquiries into your fitness to drive. With your medical questionnaire(s) you will also receive a GP claim form. It is up to your doctor to fill this in and return it to DVA if they wish to obtain payment for completion of your medical form. DVA will pay the appropriate fee to your GP (doctor) for the completion of your medical form(s) on the first occasion only. Payment for duplicates will be your responsibility. How to tell DVA about a medical condition If you've had or currently suffer from a medical condition or disability that may affect your driving you must tell Driver & Vehicle Agency (DVA). You'll also need to provide details if you develop a new condition or disability or one that has become worse since your licence was issued. Failure to notify DVA is a criminal offence and is punishable by a fine of up to £1,000.
