I would definitely contact the board houses. They will be able to give you cost
adders for each feature that is above the basice board cost such as

board size, trace width, number of layers, blind & buried vias, counterboring,
types of finish, dielectric materials(Gore,getek,nalco,fr408,etc.), copper
thicknesses, controlled impedance(differential& single ended), drill sizes, 
aspect
ratios for drills and boards,and many other features that I can't think of at 
the
present .
