 Agilent just released Part 3 of their Application note entitled The ABC's
of De-Embedding.   This is part of their SI Analysis Series.  I believe this
is covered in the application note.

Port extensions simply add in an amount of electrical length that the user
specifies.  The phase is mathematically 'un-wound' over frequency based on
that setting.  No losses or other problems are accounted for.
The assumption is a 'perfect Zo transmission line of electrical length x'.

Oftentimes it is plenty good enough.

If you need more than that, you need to de-embed the structure in question
with data from either simulation or measurements.
