Doug you must be one of the rare men who look at instructions first. I know
mine at home tries to do things and then I have to ask him has he read the
instructions and the answer is always no.

Don't try too hard to work out the truth. The rules are there for
these cases. As John says, the comma follows (LEWIS).
I know that some women are like my daughter. She came to me and asked
me how to use something she had just bought.
I asked "Where are the instructions?"
She replied "I threw them away."
She would never look up a phone book, but rang Directory Assistance.
John, that is something we men just have to live with. Don't expect
women to be men.
Doug.

Margaret,
This case is EXACTLY as described in the documentation, in relation
to the position of the comma.
John
How do I index this notice.
Do I put HARDING, William John and in the Previous name LEWIS,
William John or do I put
HARDING, William John (Lewis)
The nick names in the notice are usually in lower case and with "
either side of it in the Advertiser.
Margaret.
