Hi Andrew,

Thanks for the response, much appreciated.  I tried selecting all the text
in the document and setting the shading like you suggested to Clear with no
luck, and I selected just the form field and set it to clear as well with no
luck.  They were both already set to Clear btw.

John,

just a random thought - if the shading shows on the screen AND prints, then
this shading must have been applied from the Format, Borders and Shading
menu, NOT from the Shading option on the toolbar.  The former both shows on
screen and on the printed document, the latter shading does not print.

Try selecting these fields and making the shading Clear (0%) in the Format,
Borders and Shading dialog.
