I tried selecting all the text in the document and setting the shading like you suggested to Clear with no luck, and I selected just the form field and set it to clear as well with no luck. The former both shows on screen and on the printed document, the latter shading does not print. Try selecting these fields and making the shading Clear (0%) in the Format, Borders and Shading dialog.
