With modern spamware using traditional mail-merge techniques in a new way, it's fairly trivial to customize each spam. One (now retired) spammer related this: "I take a list of addresses and plug it into the program. I check options, including 'Use target address as From', then paste in the body of the mailing. I click 'Ok', and after a bit of processing time where it generates all of the mails, it sends them out."
