Couple notes on making your shortcut.

1:  You can press alt+enter to bring up the shortcut properties when it is
on the desktop. Alt+enter works just about everywhere except the start menu
to bring up the properties of a file or folder.

2:  You could assign the hotkey in the shortcut in the start menu.  Navigate
to the notepad shortcut in the start menu.  Press applications key.  Arrow
to properties and press enter to open the properties of the notepad shortcut
that is in the start menu.  Check to be sure it is correct before modifying
it.

3:  You only need to press the letter n when focused on the hotkey field and
windows will assign ctrl+alt+n as the hot key.

4: tab through the whole dialog and make sure the shortcut actually points
to
%SystemRoot%\system32\notepad.exe

5: Keep tabbing to find out if your hot key was actually assigned. Sometimes Jaws will read something like control+alt without showing the actual key you set.

6: Sometimes Windows gets confused.  Fore example:
I have a situation where a hotkey I've assigned doesn't always work.  In my
case I have a shortcut to my favorites folder.  I also have another
favorites folder on my system that has older bookmarks.  Somehow this
confused Windows and instead of opening my favorites folder the shortcut
opens my documents.  I think it is because favorites is a system folder.  I
couldn't even rename the second favorites folder without it changing the
name of the original at the same time.

Ok, let's try this step by step:
1. find Word/Notepad and hit the application key
2. arrow down to 'send to, create shortcut on desktop', hit enter on that.
With that you should have created an icon on the desktop now.
3. go to desktop, find the newly created icon there and hit application
key
on it.
4. arrow down to 'properties', hit enter on it.
5. tab until you hear 'key combo'. By default nothing should be in there.
When you have focus on that field press alt + control + n together.
6. tab further until you hear 'apply'. This is past the ok button. On
'apply' hit enter and then 'ok' and the window then close.
