This posting says it pretty well.
J-Say is produced by T&T consultancy and when you buy it, you get Dragon speech 
recognition software + Headset mic/earphone + the scripts and macros to enable 
JAWS to work well with the Dragon software.

There are two versions of J-Say, standard, which comes with Dragon Preferred, 
the standard version of Dragon that offers good recognition but relatively 
limited control of the computer's other functions.  The J-Say Professional 
version comes with much more control of other functions of the computer.

In which country are you located?  If you are UK, then go direct to T&T, if in 
the US, try Chirping Bat who is a J-Say dealer there.
