Thanks so far for all the feedback. I've tried simplifying the setup but
still no luck.

The steps I've taken include:
- Disabling the PF
- My destination set includes all incoming addresses
- My port allocation for internal request is the same as the IIS ports
- I've disabled Socket pooling
- Incoming Listener set with standard ports.

Any help would be great this is becoming a real pain!

Hi Rafael,
First, when you publish (server or web), there is no need to create a
packet
filter to allow the external connections,
The ISA will open automatically the needed ports.
Second, If your external IP is DHCP feeded, check the publishing rule
uses
that IP.
There is a script that updates you publishing rule
