Q1 - how many external IPs does the ISA have? Q2 - if more than one, have you disabled the Incoming Web requests listener for the IP your server-published IP? The issue is that even if you server-publish HTTP, if the Incoming Web requests listener is enabled on the IP, it gets priority (it's smarter that any SPR).
