Hi Gillian,
 
So, outbound mail is working? That is to say, mail from the internal
network is finding its way to the Internet?
 
And, inbound mail from the Internet is not making its way to your
internal network?
 
Or, is it the other way around?

        Thanks Tom.  Very helpful info and some progress.  But...
         
        I've now got external mail flowing using the SMTP DMZ relay
server fine but inbound is still not working.  I understand from your
post that external mail flow is from (among other things) the SMTP
Virtual server config and internal mail flow from the Remote Domains
config on the SMTP server.
         
        What's the best way to troubleshoot this internal mail flow
issue?  I've double-checked the ISA server publishing rules on both
ISA's and they look correct.  I've double checked the Remote Domains
config which forwards to the Internal ISA's external interface.
         
        So close but yet so far...  :)
