For me, it depends on the type of book and text.  If there are references to
those pages or sections with the exact letter number combinations, I do
this:

page break
blank line
consecutive page number from previous pages that were numbered blank line
given page/section number (if the book has nothing in 14 pt, then I 14 pt
and bold it) blank line page contents

If there are no overt references to those page numbers elsewhere, I skip
the:

given page/section number (if the book has nothing in 14 pt, then I 14 pt
and bold it) blank line

Not sure if this is kosher, but how I handle it ...
