HI Kasondra, I would agree with Gerald in that it is what the author published and we are not to make judgments or changes to text. As far as the form goes I have seen this before when a submitter only includes one author or editor. This to me is a part of the validators job to check this info and correct it if needed. Just because it was over looked is no reason for rejection. Honestly, I am sure there are many books that miss represent history or facts to support a particular view.
