Yes, you are correct that the extraneous page breaks should be removed. It
seems to depend on what software you are using to proofread as to why the
extra page breaks are there and how best to get rid of them. You are also
correct about what the page headers are and, yes, they should be removed. After
you finish with a page it should look like this: page break, blank line,
page number, blank line, text of page, blank line, page break.
