Hi Theresa.  the Document is quite accessible with Zoom  Text.  The only thing 
that it did not read was the presence of the tick boxes.  As Zoom Text is meant 
for people with some vision this does not present an insurmountable problem.  
It is as you thought clearly a reproduction of the printed form.

  In my opinion, the online version has been developed to look very much like 
the paper version - but I'm not sure it works online.  I'll reserve any further
