We use a couple of conservative blackhole lists, then run spamassassin (with the Vipul's Razor and Bayes modules), and anything that scores too high is run through TMDA.

My AOL account has been getting alot more, however our work addresses seem to be nearly impervious. So far my three tier anti-spam system is working very well.
