Not sure how accessible" Survey Monkey" is but I have answered several survey's
that have been sent to me as a VRT and I would think others in the field with
visual impairment. Don't know how successful the accessibility has been.

Could someone recommend a site where someone could set up a survey, that would
be accessible for the survey setter uper and the participants?
