Jerry, in BANA countries, we use the 'Braille Formats' document for
formatting specifications.
You will find BRF and accessible PDFs of this document at the following URL.

Headings are described in section 4. Happily, each section of 'Braille
Formats' is saved in an independent BRF file so you have only to look for
the BRF named 04_Headings.brf in the individually zipped files available at
the URL noted above.
