Dont know that much about csg/isa, but could they not use port mappings on the
net router itself? (presumably theres a box upstream of the isa server?)

Has anyone ever setup a Secure Gateway implementation with ISA server as
the main perimeter firewall device without assigning a registered
address space to the DMZ?

I have a client who is in the situation where they cannot obtain any
additional IP addresses from their ISP, so I can't setup a DMZ scenario
on the ISA server and have it available publicly, as ISA doesn't perform
NAT on a DMZ.

Any pointers would be appreciated.
